This session breaks down the key steps involved in accessing disability services, from qualification and assessments to annual planning and ongoing support. Participants will leave with a clearer understanding of the process and the people who can help guide them along the way.

  • Learn how to prepare for a MnCHOICES Assessment
  • Recognize the importance of the annual planning process
  • Identify your support team for accessing services, including the lead agency, case manager, and service provider
