About CFSS Consultation Services
CFSS Consultation Services are a mandatory resource designed to help Minnesota participants navigate their home care options by providing unbiased education on the Agency and Budget models. These services assist individuals in developing and finalizing their personalized Service Delivery Plan to ensure their care aligns with their specific goals and needs.
CFSS participants choose a Consultation Services provider to support them with learning about the models of CFSS, developing and reviewing their plan, and ongoing support throughout the year. Lifeworks is proud to be an approved contracted consultation services provider for the state of Minnesota.Â
Eligibility for CFSS Consultation Services
To be eligible for CFSS Consultation Services, an individual must be enrolled in a Minnesota Health Care Program and have a state-approved assessment confirming a need for assistance with daily activities or behaviors. Current participants of the legacy PCA or CSG programs gain access to consultation services during their annual reassessment as they transition into the CFSS program.
