Minnesota Care Force Incentive Grant Distribution Plan
- In September 2024, Lifeworks Services, Inc. applied for incentive payments on behalf of DSPs through the Minnesota Care Force Incentive program.
- In order to retain and reward DSPs for their dedication and role in providing high quality care, Lifeworks expects to receive up to $1,000 per eligible employee submitted on each list provided in Phase 2. The final amount approved by DHS was $770.
- Our distribution goal will be to reach as many DSPs as possible. Eligible employee means any employee who makes less than $30 per hour and is actively working for at least one Lifeworks enrolled Minnesota Health Care Program on the date of payment distribution.
- Regardless of whether a DSP is new or has been providing services for some time, retention and reward payments will be distributed in equal amounts.
- Each eligible employee will only be eligible for one payment by Lifeworks, regardless of the number of programs they are working under and must be on the approved list.
- Employees who DHS determines to be ineligible will be excluded from the distribution.
- Once Lifeworks receives the award, Lifeworks will notify all DSPs of the distribution plan.
- Payments to DSPs will be distributed within 45 days or sooner to each eligible employee.
- If any employee no longer works for Lifeworks on the payment date, Lifeworks will not distribute the incentive payment to the former DSP. Lifeworks will indicate this in the final distribution report and will return the unpaid incentive funds to DHS.
- Lifeworks will fully distribute $770 less required employee and employer state and federal taxes and insurance (exemptions may apply) equally to each eligible employee.
- Within 90 days of the distribution, Lifeworks will submit a final Distribution Report to the Minnesota Department of Human Services.
- Lastly, within 12 months of receiving the incentive payment, Lifeworks will return any undistributed funds to the Minnesota Department of Human Services.
