What are Individualized Home Supports, Respite, and Night Supervision?
Individualized Home Supports/Personal Support is a one-to-one service that provides opportunities for people to increase their independence and connect with their community. Support can include assistance with daily living activities such as eating breakfast or brushing teeth to participating in local events and building relationships with friends. Respite complements this service and provides short-term relief for a person’s primary caregiver.
Night Supervision is a one-to-one service that takes place in a person’s home. Staff remain awake overnight and assist with following a person’s plan, encouraging skill development, and helping with activities of daily living (ADLs and IADLs). Family and friends can be hired as support staff, providing relief for primary caregivers. Night Supervision can be used for up to 12 hours in a 24 hour period and can complement Personal Care Assistance (PCA) Choice as well as Individualized Home Supports/Personal Support and Respite Services. Am I eligible to receive these services?
As 245D Basic Services, these types of support require that a person has a waiver for either Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI), or Developmental Disabilities (DD), and is referred by a case manager.

Jodi Aschoff
Jodi Aschoff joined Lifeworks in July 2019 as a Service Coordinator. She earned her Bachelor’s Degree in Human Services and has a minor in disability studies. She also holds an Associate of Applied Science Degree in Early Childhood Development. Prior to Lifeworks, Jodi was a director at a leading childcare center in the Twin Cities.
Jodi lives in Burnsville with her three children. Her personal experience with people who have disabilities have led Jodi to actively work to make her community an inclusive space for everyone. In her spare time Jodi enjoys listening to music, camping, spending time on the water and being with friends and family.
Leah Swenson
Leah Swenson began working at Lifeworks in May of 2021. Leah has a long history of serving individuals with disabilities. Some of Leah’s past work experiences include managing group homes, providing direct care, and waiver case management. Leah and her husband have four children. Leah and her family live on a small hobby farm with animals and a large garden. When Leah is not working, she enjoys baking, sewing, and attending 4H activities with her kids.
Emalee Irwin-Roeske
Emalee Irwin-Roeske joined Lifeworks in August of 2021. Emalee has a Bachelor’s Degree in Child Adult and Family Services from Iowa State University with a minor in Family Finance and Housing Policies. Prior to Lifeworks, Emalee was the director over several group homes for adults and teens with disabilities.
Emalee lives in Prior Lake with her husband and their three cats. In her spare time Emalee enjoys hiking, games, escape rooms, and knitting.
Jolene Easterday
Jolene Easterday joined Lifeworks in February of 2022. Jolene has an Associates of Science in Human Services degree and has worked in disability services in a variety of settings for over 10 years. Jolene has worked in group homes, independent living apartments, day support programs as well as other support roles for providers prior to joining Lifeworks.
Jolene grew up in southeastern North Dakota and moved to Minnesota in 2013. Jolene currently lives in Burnsville with her fiancé and future mother-in-law. Jolene has 2 dogs and 2 birds at home. In her spare time, Jolene enjoys relaxing in the sun, watching hockey and cooking.
